ATHLETIC PHYSICALS COLLECTION DATES
For this summer the planed collection dates for the physicals will be: JULY 15th, 29th AUGUST 5th,12th,14th.. They need to be turned in by 10:00 am on these dates.
There will a collection box in the Office of the HIGH SCHOOL. Extra physical forms, for the high school and middle school, will be in the high school office.
If the athlete mails them in – they need to be marked PHYSICAL and Attn: Athletic Trainer ( not the nurse)
Athletes should be sure ALL information is filled out on all the forms, and all forms returned together.
It is highly recommended that the PHYSICAL PACKAGES be received by August 1st to insure that your physical is cleared on time for the start of practices.
